Privacy Notice

Last updated: September 3, 2026

This notice covers the website only. It explains what we ask for and why, and what we deliberately leave out. Health information about our patients is governed by a separate document.

The short version

  • Nothing on this site tracks you. There is no analytics platform and no advertising software.
  • The only cookies we set sign in our own staff. As a visitor you are given none.
  • Our referral form does not ask for diagnoses, medications, insurance numbers, or Social Security numbers.
  • We never sell or rent your information.

Health information about our patients is governed by our Notice of Privacy Practices, which explains your rights under HIPAA.

Information we collect

We only collect what you type into one of our forms. Browsing the site leaves no profile behind.

Patient referral form
Name, phone number, email address, city, state, ZIP code, county, the services you are asking about, who is making the referral and their relationship to the patient, and any notes you add.
Contact form
Name, email address, phone number, and your message.
Job applications
Contact and address details, the position sought, availability, education, licenses and certifications, employment history, references, work authorization, driving and transportation answers, criminal-conviction disclosure, your consent to a background check and drug screening, and any resume you upload.
Staff portal
Only our own employees can sign in. Their sign-in creates a session cookie. Members of the public have no account and no way to read submissions.

What we deliberately do not collect online

Our referral form is built to take as little health information as possible. There is nowhere on it to record a diagnosis, a medication list, allergies, a date of birth, a Social Security number, a Medicare or Medicaid number, an insurance member ID, your physician's details, or your street address. Those fields do not exist.

Clinical and insurance details are gathered later, by phone or fax, once a coordinator has reached you. Please do not type them into any form on this site. If you do, we will remove them.

How we use your information

Referral and contact submissions are used to call you back and arrange care. Job applications are used to evaluate you for the role you applied to.

That is all. Your information is never used for advertising, and we send no marketing email.

Cookies and tracking

This site runs no analytics platform and no advertising pixel. Nothing here records your session or builds a heat map. There is no cookie banner because, as a visitor, you are given no cookies to consent to.

The only cookies we set belong to the staff portal, and they exist to keep a signed-in employee signed in. They are strictly necessary, and members of the public never receive one.

One exception is worth naming. Our contact page embeds a Google Map, and when it loads, Google receives your IP address and may set its own cookies under Google's privacy policy rather than ours.

Who we share it with

Your information is never sold, rented, or traded. It goes only to the service providers that make the site work, and only as far as they need it:

  • Our database and file-storage provider, which keeps submissions on servers in the United States.
  • The email service that delivers notifications to our staff and questionnaire links to applicants.
  • Our website host.

The law can also require disclosure, and we may disclose information to protect someone's safety.

How we protect it

Traffic to this site is encrypted. Submissions land in a database where row-level security blocks public reads outright: you can send a form, but you cannot read anything back, not even your own submission.

Questionnaire links sent to applicants expire, and each one works once. Only a cryptographic hash of the link is stored, so a copy of our database hands nobody a working link.

No system is perfectly secure. If a breach affects your information, we will notify you and the appropriate authorities as the law requires.

How long we keep it

Patient clinical records are kept for at least five years after the date of last discharge, the period Texas home health licensing rules and the Medicare Conditions of Participation require. Where the patient was a minor, records are kept until the patient's twentieth birthday or five years after discharge, whichever is later.

Referral and contact submissions that never become patient records are kept for twenty-four months and then deleted. Applications from candidates we do not hire are kept for one year, the minimum federal employment rules require. Personnel files for staff we do hire are kept for the length of employment plus five years.

Your choices

Ask what we hold about you and we will tell you. You can also ask us to correct it or delete it, using the contact details below.

Patients have further rights over health information under HIPAA. Our Notice of Privacy Practices describes them.

Job applicants

Applying means consenting to a background check and, where applicable, drug screening. You give that consent on the application form itself, and neither is run without it.

What you submit is used to assess your suitability for the role. It goes nowhere outside the hiring process, except to the providers named above.

Children

This website is not directed at children, and we do not knowingly collect their information through it. Where a patient is a minor, a parent or guardian provides the details.

Changes to this notice

If we change this notice, the date at the top changes with it. Anything material will be flagged here.
